A mechanic in a shop that builds MOW specialty vehicles was lifting an articulating crane to mount it on a truck bed when the sling failed. The mechanic was fatally injured when the load fell to the ground and toppled on top of him. The sling was not protected at the corners and was not arranged to prevent it from sliding during the lift. The crane was articulated during the lift to shift its center of gravity. The articulating crane manufacturer reached out to ESi for help. The codefendants included the rigging equipment manufacturer, distributor, and others.

On May 22, 2018, a two-year-old boy became trapped between the bunk bed upper bunk and the top rung of the ladder at his residence in Ohio.

The plaintiff was returning from a hunting trip in a 2007 Ford E-350 camper van when the left rear axle fractured, and the wheel suddenly separated from the vehicle. A loss of control rollover accident followed, leaving the passenger with a fractured cervical spine injury and permanent paralysis.
